Ang\u00e8le Mal\u00e2tre-Lansac is Director of Health at the Institut Montaigne. The aim of the Health programme she leads is to bring together all the stakeholders in the healthcare sector: professionals, patients, industry, researchers, payers, senior civil servants, healthcare providers, etc., in order to decipher, anticipate, compare and propose ways of changing the face of our healthcare system, drawing on the best French and international examples.<\/p>\n
\"The Covid-19 pandemic highlights both the strengths and weaknesses of our healthcare systems. In all OECD countries, the proliferation of chronic diseases, ageing populations and technological developments are challenging the sustainability of our systems. Healthcare spending continues to rise (17 % of GDP in the United States, 12% in France), but the value created for patients does not seem to be keeping pace, with the rise of medical deserts, innovations that are less and less accessible, care that is still not very personalised, and little prevention. If spending is increasing without creating any additional value, or only a little, it's because the spending is not well directed and the incentives sent to healthcare players are not the right ones. Studies show that almost 30% of healthcare expenditure is ineffective, or even harmful, while access to the latest innovations remains limited for the majority of patients. <\/p>\n
How can we change the paradigm and ensure that our healthcare systems encourage the creation of more value for patients (value-based healthcare) and avoid unnecessary acts and waste? At a time when we are going through an unprecedented health crisis, it is more essential than ever to focus our efforts and spending on the quality of care and innovation, and to stop remunerating healthcare professionals on the basis of the number of procedures they perform, without taking their performance into account.\u00a0<\/p>\n
The good news is that we now have accurate data with which to measure the value created by healthcare players, as well as the means to interpret it thanks to the development of artificial intelligence. Value-based healthcare is made possible by the collection of real-life data and data directly from patients. Have you ever been asked, after an operation, whether the results matched your expectations? If your quality of life was satisfactory? How did you feel about the experience? These data, known as Patient Reported Outcomes (PROMs) and Patient Reported Experiences (PREMs), are essential for measuring the quality of care and innovations, and for ensuring that every player in the healthcare sector is creating value for patients.<\/p>\n
Health data, whether clinical, genomic, from connected objects, mobile applications, PROMs or PREMs, must be at the heart of the debate on the management of healthcare systems. Thanks to this data, we will be able to remunerate the players involved and incentivise them to improve quality, enable more agile evaluation of innovations, encourage collaboration between the players involved and give patients the information they need to find their way around and choose their healthcare pathway according to their needs and preferences.<\/p>\n
